  • The Care Coordinator serves as the primary liaison between patients, families, and the healthcare team. This role optimizes clinic workflow by managing care plans, facilitating smooth discharges, and ensuring patients are connected with the appropriate medical and community resources.
  • Must have good communication skills
  • Patient Advocacy & Care Planning:
    • Consult with patients and Clinician to assess needs and build tailored care plans.
    • Educate patients on their conditions, medications, and treatment instructions. 

  • Administrative & Workflow Management:
    • Schedule appointments, verify insurance coverage, and handle intake documentation.
    • Track test results, medication lists, and follow-up
